Flu consents were distributed in school and are due on September 21st. If your child did not receive a flu consent, you can go online and complete the consent there, www.vaccineconsent.com
The flu clinic will be held on October 9th, Tuesday, from 7:30am-9:30am. There will be no make-up dates. If needed, you can make an appointment with your child's doctor or contact your local pharmacy for his/her flu shot. If you are unable to complete the consent online, attached is the consent we sent out. There is an English version and Spanish version.
