Immunizations must be up to date.  Per Maryland Department of Health and Mental Hygiene, all 9th graders must have proof of immunizations of the Tdap and MCV4.  This was implemented for the new school year 2016-2017.  Please send in proof of immunizations to the Health Room.  The Howard County Health Department is providing immunizations by appointment only.  See information below:

Please send in current medications orders to the Health Room for your student.  They must be dated after July 1st, 2016, and be signed by the doctor and parent/legal guardian. 

If student is to be taking a prescribed over-the-counter medication, the bottle or package must be unopened/sealed for the Health Room to accept it.

Please send in any requested health forms such as physical examinations, health history forms, and doctor's notes for any activity restrictions.
