Due to lower-than-expected numbers for our upcoming flu clinic on Tuesday, October 3, 2017, we have decided to extend our deadline for consents until September 28, 2017, in order to give families more time to complete and return consents. Your children will all be bringing home paper copies of the consent on 9/25 or 9/26. Please ask your child for the consent. Extra copies will also be available in the Health Room. IF YOU HAVE ALREADY COMPLETED AN ELECTRONIC OR PAPER CONSENT, you do not need to fill out another one. If you haven't yet filled out a consent and would like your child to be vaccinated against the flu, please fill out either the paper consent that comes home with your student or the online consent at www.vaccineconsent.com by Thursday, September 28th -no consents will be accepted after the due date. Influenza, also known as the flu, is a serious illness that affects people of all ages. One of the best ways to prevent the flu is through vaccinations. We are pleased to again join with the Maryland Partnership for Prevention (MPP) to offer flu vaccinations at our school.
New this year, you may give permission for your child to be vaccinated using (1) an online form at www.vaccineconsent.com or (2) the hard copy form attached to this message.
We urge you to protect your family from the flu. The Centers for Disease Control and Prevention (CDC) recommends that everyone ages 6 months and older get a flu shot. Please visit www.cdc.gov/flu/index.htm for more information about the flu vaccine.
The clinic at our school will be held on October 3th. We will send a notice home the day he/she has been vaccinated. If you want your child to be vaccinated, please:
Fill out every section of the attached Consent Form OR complete it online at www.vaccineconsent.com. If you fill out the paper consent, write neatly and use ink. Only fill out the hard copy consent or the online consent. Do NOT fill out both.
Use your insurance card to fill out your insurance information accurately. Your insurance company will be billed. There is no co-pay or deductible. You will NOT be charged by us or your insurance company.
Read the Vaccine Information Statement (VIS) and talk to your health care provider or the school nurse about any questions you have.
Return the consent form no later than September 20, 2017.

La influenza, o gripe, es una enfermedad grave que afecta a personas de todas las edades. Una de las mejores maneras de prevenir la gripe es con la vacunación. Tenemos el agrado de asociarnos a Maryland Partnership for Prevention (MPP) para ofrecer la vacunación antigripal en la escuela de su hijo.
Le recordamos que se trata de una vacuna antigripal inyectable. Esto significa que se inyectará en el brazo de su niño a través de una pequeña aguja. La vacuna contra la gripe es muy segura.
